Midstream College Application Form 2024-2025: School Online Applications Requirement

Midstream College has reopened the application portal for all parents who are welcome to admit and submit their children’s application details to the school for the academic year 2024-2025. The school has made available an application form that allows pupils to fill out while providing the relevant data about themselves, their parents/or guidance. Before submitting it before the deadline expires, applicants should also include the necessary documentation.

Required documents needed to be submitted

Please ensure that you have all the following documentation ready in pdf or jpeg format before you apply online (scan as individual documents and name accordingly):

• Copy of learner’s birth certificate (preferably unabridged);
• Colour photo of learner (passport sized – an official school photo of 200kb or less is preferable);
• Copy of parents’ identity documents;
• Proof of residence (e.g. electricity account);
• Proof of payment (non-refundable admin fee of R500);
• Details for finance department (MC002E);
• Signed terms and conditions form (MC003E);
• Any legal documentation regarding the learner (if applicable – proof of custody/proof of sole parent);
• The latest report from the previous school;
• Financial clearance certificate from the previous school (MC004E). (Midstream learners – attach the latest school statement in place of the MC004E.);
• Financial Policy (MC 004B);
• Subject choice form (MC033EN – for grade 10 and 11 applications only).

Please note you must complete the online process as you cannot save and return to the form at a later stage.

 

How to submit the application form to Midstream College

Please follow the steps below to upload your documents to the school during the application period:

• Visit the school’s online application portal’s website.
• Select “Load Documents”
• After descending, click the bubble next to “Documents”
• Fill up your details in the provided space given
• Select the matching file from your device by clicking the choose file button.
• Click “Upload.”
• After submitting the documents, click “submit documents” to move forward.

If you followed the steps above, you could successfully upload your documents to the school without any problem. Students should anticipate being accepted into the institution for the 2024–2025 academic year.
