Roosevelt High School Application Form 2024-2025: Opening Date for Applications

All candidates interested in attending Roosevelt High School for the academic year may apply beginning with the academic year 2024–2025. The application form for the school has been provided in the link below. All applicants should fill it out completely, attach the correct documentation, and submit it.

REQUIRED DOCUMENTS TO ACCOMPANY THE ONLINE APPLICATION

The following document is required to complete the online application:

• Fully completed and signed “Additional Information” document.
• GDE reference number (https://www.gdeadmissions.gov.za/Home/Index).
• Certified copy of the UNABRIDGED Birth Certificate of the learner as well as the original for verification purposes.
• Certified copy of the latest report.
• Certified copies of both parents’/legal guardians’ ID documents. As well as the originals for verification purposes.
• Certified copy of Divorce and Maintenance Agreement (if applicable).
• Certified copies of death certificate if parent/s is/are deceased.
• Certified copy of the Court Order granting Legal Guardianship (if applicable).
• Proof of residence in our area: COJ utility bill/Deed of Sale (NO exceptions will be made). If renting: a copy of the lease agreement plus the utility bill and ID document of the Lessor
• Proof of permanent work address for both parents/legal guardians. With a salary slip AND bank statement for BOTH parents into which the salary is paid.
• Certified copies of current passports for parents/legal guardians and learners (Immigrants only).
• Certified copy of the learner’s current Study Visa (Immigrants only).
• No faxed or e-mailed documents will be accepted.

Should any of the documentation submitted be found to be fraudulent, they reserve the right to cancel the application.

INCOMPLETE APPLICATIONS WILL NOT BE ACCEPTED!

 

How to submit the application form to Roosevelt High School

Please follow the steps below to upload your documents to the school during the application period:

• Visit the school’s online application portal’s website.
• Select “Load Documents”
• After descending, click the bubble next to “Documents”
• Fill up your details in the provided space given
• Select the matching file from your device by clicking the choose file button.
• Click “Upload.”
• After submitting the documents, click “submit documents” to move forward.

If you followed the steps above, you could successfully upload your documents to the school without any problem. Students should anticipate being accepted into the institution for the 2024–2025 school year.
